Registration Forms

PARENT DIRECTED and SHARED RESPONSIBILITY students will need to complete the following forms for registration.

1. PowerSchool Registration. A Birth Certificate and proof of residence will also need to be provided for registration.
2. Home Education Notification Form for supervised Learning
3. Technical Requirements. Please read this to see what you will need to have available for the upcoming school year.
4. Program Plan Template – Parent Directed families are required to complete & submit a Program Plan before the last school day in September.  This plan should be started prior to the Program Planning meeting with the HBS Coordinator.  Families may use the provided template or design their own that meets the requirements.

TEACHER DIRECTED Families will need to complete and return the following forms for registration.

Once these forms have been received, a Program Plan meeting will be arranged with the Home-Based Coordinator.
